Gary Thomas and Jeanne live in Western Australia. He was 58 when he was diagnosed in August, 2013. His initial PSA was 6.90 ng/ml, his Gleason Score was 9, and he was staged T4. His initial treatment choice was External Beam Radiation+ADT (Intensity Modulated with ADT) and his current treatment choice is None. Here is his story.

I was not suitable for surgery as the cancer had spread to the lower spine (L5).

I have no significant long term effects from ADT, EBRT and chemotherapy (Docetaxel).

PSA has remained at 0.02 for last two 3 monthly tests.

I have an excellent quality of life with no pain or fatigue despite ADT.

PSA rose gradually to 1.21 over about a year.

EBRT given to a new pelvic lymph node met. MO said this would be better than bombarding me with more ADT & chemo.

PSA now 1.07.

PSA rose slowly from <0.02 to 1.21 over about a year.

On the 12th of December 2018 I completed 40 Gy in 5 fractions of stereotactic radiation to a presacral lymph node. PSA has continued to fall slowly over about a year to 0.02.

I have a good quality of life with no pain and no significant long term effects.

I am meeting with a urologist for a flow test, possible biopsy and Space OAR gel and re-radiation of the prostate. I am pain free and have enough energy for bike rides.

Good quality of life but no libido. Some hot flushes.

My PSA is gradually rising. Last scans showed nothing. Medical oncologist will re-test PSA and Testosterone on 18th Dec 24. If anything shows, spot radiation.
